评价窄带成像技术(narrow-band imaging,NBI)结合显微放大胃镜对于诊断幽门螺杆菌(Helicobacter pylori,Hp)感染的价值。

方法

通过检索PubMed、Cochrane Library、EMbase、中国知网等数据库2011年1月至2016年5月有关NBI放大内镜诊断Hp感染的诊断性试验,研究文献严格根据纳入及排除标准筛选文献,对于符合纳入标准的文献提取所需相关数据,使用Meta-DiSc软件进行Meta分析。

结果

纳入文献共有6篇,包括受试患者共870例(940处病灶),NBI显微放大内镜诊断Hp感染的总灵敏度为:0.89(95% CI:0.86~0.92),总特异度为0.81(95% CI:0.77~0.85),阳性似然比为:4.90(95% CI:2.25~10.68),阴性似然比:0.11(95% CI:0.04~0.31),诊断优势比:47.55(95% CI:11.84~190.92),曲线下面积为:0.922,Q*指数为0.856。

结论

NBI结合显微放大内镜对Hp感染的诊断具有一定的价值。

Objective

To evaluate the diagnostic value of narrow-band imaging(NBI)with magnification gastroscopy in Helicobacter pylori(Hp)infection.

Methods

Literatures on NBI with magnification gastroscopy in diagnosing of Hp infection were searched in PubMed, Cochrane Library, EMBase, China National Knowledge Infrastructure and so on.According to the inclusion and exclusion criteria, the most suitable literatures were selected.The Meta-DiSc software was used to review and analyze.

Results

There were 6 studies met our inclusion, including 870 patients(940 areas). The pooled sensitivity and specificity of NBI with magnification gastroscopy in Hp infection were 0.89(95% CI: 0.86-0.92)and 0.81(95% CI: 0.77-0.85)respectively, positive 1ikelihood ratio was 4.90(95% CI: 2.25-10.68), negative likelihood ratio was 0.11(95% CI: 0.04-0.31), and the diagnostic odds ratio was 47.55(95% CI: 11.84-190.92). The area under the curve was 0.922, and Q* was 0.856.

Conclusions

The NBI with magnification gastroscopy is of definite value for the diagnosis of Hp infectione.
