目的

探讨幽门螺杆菌(Hp)感染与基于"生命简单七项"(LS7)的心血管健康评分之间的关联,并分析系统性炎症的中介作用及性别调节效应。

方法

本研究为回顾性设计,纳入2023年12月至2025年12月于北京医院就诊的Hp感染组与无Hp感染组受试者各206例。收集人口学资料、LS7各组分评分、血液生化及炎症指标。使用多元线性回归分析Hp感染与LS7总分的独立关联,采用结构方程模型检验超敏C反应蛋白(hs-CRP)的中介作用,并通过有调节的中介模型分析性别差异。

结果

Hp感染组的LS7总分显著低于非感染组(8.2±2.1 vs. 10.5±2.0,P<0.001),且Hp感染率随LS7健康等级下降呈阶梯式上升(理想组28.1%,中等组50.5%,差组75.0%)。中介分析显示,hs-CRP部分中介了Hp感染对LS7评分的负面关联,中介效应占比约38%(标准化回归系数β=-0.38,P<0.001)。调节效应分析表明,Hp感染对男性心血管健康的损害效应(β=-0.955)显著强于女性(β=-0.875)。

结论

Hp感染与较差的心血管健康状态显著相关,系统性炎症是其间的重要中介途径,且该关联存在性别差异,对男性的负面影响更为突出。这提示在心血管健康管理中,对Hp感染者进行炎症状态评估与综合干预具有潜在意义。

Objective

To explore the association between Helicobacter pylori (Hp) infection and cardiovascular health scores based on the "Life's Simple Seven" (LS7), and to analyze the mediating effect of systemic inflammation and the moderating effect of gender.

Methods

This study was designed as a retrospective study, enrolling 206 subjects each from the Hp-infected group and non-Hp-infected group who visited Beijing Hospital from December 2023 to December 2025, and demographic data, LS7 component scores, blood biochemical indicators and inflammatory markers were collected. Multivariate linear regression analysis was performed to assess the independent association between Hp infection and total LS7 score. Structural equation modeling (SEM) was utilized to investigate the mediating role of high-sensitivity C-reactive protein (hs-CRP), and gender differences were analyzed via a moderated mediation model.

Results

The total LS7 score was significantly lower in the Hp-infected group than that in the non-Hp-infected group (8.2±2.1 vs. 10.5±2.0, P<0.001), and the Hp infection rate increased progressively as LS7 health grades deteriorated (28.1% in the ideal group, 50.5% in the moderate group, and 75.0% in the poor group). Mediation analysis demonstrated that hs-CRP partially mediated the adverse effect of Hp infection on LS7 scores, contributing to approximately 38% of the total effect (standardized β=-0.38, P<0.001). Moderation effect analysis showed that the detrimental effect of Hp infection on cardiovascular health was significantly more pronounced in males (β=-0.955) than that in females (β=-0.875).

Conclusion

Hp infection is significantly associated with suboptimal cardiovascular health, and systemic inflammation acts as a crucial mediating pathway. This association is characterized by gender differences, with more marked adverse effects in males. This implies that evaluating inflammatory status and implementing comprehensive interventions for Hp-infected individuals may be of potential value in cardiovascular health management.
