无痛内镜下修补十二指肠球部溃疡穿孔一例

无痛内镜下修补十二指肠球部溃疡穿孔一例

患者男性，68岁，主因"黑便9天，腹痛1 h"于2022年10月19日入院。患者9 d前无明显诱因出现黑色糊状大便，2~4次/d，无呕血，无腹痛、腹胀，偶有头晕、心慌，活动后加重，无胸闷、气喘，无咳嗽、咳痰，无尿频、尿急、尿痛。就诊于当地卫生院，对症治疗后大便逐渐转黄。为进一步诊治于我院就诊，门诊10：40普通胃镜检查显示：十二指肠球部变形，憩室样改变，球部大弯偏前壁可见一直径约为1.5 cm溃疡，底深，中央覆盖黑色血痂，其周围为黄白苔，黏膜充血水肿，降部未见异常。约11：00患者突发上腹部撕裂样疼痛，持续加重，蜷缩位稍缓解，遂急诊以"腹痛"收入消化科。
