目的

研究肠道准备对无痛胃镜检查黏膜清晰度和安全性的影响。

方法

2019年9至10月邹平市人民医院接受无痛胃镜和肠镜健康查体的某单位员工168例，随机分为观察组和对照组两组，观察组90例术前进行肠道准备，胃肠镜在一次麻醉中完成；对照组78例先行无痛胃镜检查，择期再行肠道准备和无痛肠镜检查。对比两组胃镜检查时胃底、胃体、胃窦黏膜清晰度评分和总评分，记录并比较两组患者术中麻醉异常和术后不良反应的发生率。

结果

观察组胃底、胃体、胃窦各部位黏膜清晰度评分和总评分均低于对照组：(1.54±0.56)分vs.(2.01±0.73)分、(1.67±0.57)分vs.(1.99±0.67)分、(1.40±0.58)分vs.(1.63±0.61)分、(4.32±1.21)分vs.(5.63±1.32)分，差异有统计学意义(P<0.05)；两组患者术中麻醉异常和术后不良反应的发生率差异均无统计学意义(P>0.05)。

结论

服用聚乙二醇(polyethylene glycol，PEG)电解质散进行肠道清洁准备可提高胃黏膜清晰度，且不会增加无痛胃镜检查的麻醉风险。

Objective

To explore the effect of bowel preparation on gastric mucosal visibility and safety in painless gastroscopy.

Methods

From September to October 2019, 168 employees of a unit who received painless gastroscopy and colonoscopy were randomly divided into two groups: the observation group(n=90)received bowel preparation before operation and the gastrointestinal endoscopy was completed in one anesthesia; the control group(n=78)received painless gastroscopy first, followed by elective bowel preparation and painless enteroscopy.The visibility score and total score of gastric fundus, gastric body and antrum mucosa during gastroscopy were compared between the two groups, and the incidences of intraoperative anesthesia abnormalities and postoperative adverse reactions were recorded and compared between the two groups.

Results

The mucosal visibility score of fundus, body, antrum and total score in the observation group were lower than those in the control group: (1.54±0.56)points vs.(2.01±0.73)points, (1.67±0.57)points vs.(1.99±0.67)points, (1.40±0.58)points vs.(1.63±0.61)points, (4.32±1.21)points vs.(5.63±1.32)points, respectively; and the differences had statistical significance(P<0.05); the differences in the incidence rate of intraoperative anesthesia abnormalities and postoperative adverse reactions between the two groups had no statistical significance(P>0.05).

Conclusion

Bowel cleansing preparation with polyethylene glycol electrolyte powder can improve gastric mucosal visibility without increasing the anesthetic risk of painless gastroscopy.
