目的

评估移动远程医疗干预在功能性便秘(FC)患者中的应用效果,探讨其对便秘症状、心理健康、生活质量及患者满意度的改善作用。

方法

本研究为前瞻性、随机对照试验,选取2022年2月至2024年2月首都医科大学附属北京康复医院门诊的150例FC患者,按照随机数字法分为干预组(基于移动远程医疗平台的个性化远程管理)与对照组(常规的门诊治疗),每组75例。评估干预前后便秘症状评分、焦虑自评量表(SAS)、36项健康调查简表(SF-36),干预后的改善效果以及患者满意度。

结果

干预后,干预组患者排便频率、自主排便、便意、排便时间等方面的评分均较对照组更低(P<0.05);干预组患者干预后的SAS得分为(32.5±9.3)分,明显低于对照组的(37.8±10.4)分(P<0.05);干预组在躯体功能、社会功能、角色功能、情绪健康等维度得分显著高于对照组(P<0.05)。干预组者产生便意的时间、排便时间明显较对照组更短,有效的患者比例更高(P<0.05)。满意度方面,干预组在平台易用性、治疗效果、个性化服务、技术支持等评分均显著高于对照组(P<0.05)。

结论

移动远程医疗干预在FC患者的远程管理中具有显著效果。通过个性化的干预措施,能够有效改善便秘症状、提升心理健康、提高生活质量并增加患者的整体满意度。

Objective

To evaluate the effectiveness of mobile telemedicine interventions for patients with functional constipation (FC) and to examine its impact on constipation symptoms, mental health, quality of life, and patient satisfaction.

Methods

This study was a prospective, randomized controlled trial. A total of 150 FC patients from the outpatient department of Beijing Rehabilitation Hospital Affiliated to Capital Medical University were selected between February 2022 and February 2024. They were randomly divided into an intervention group, which received personalized remote management through a mobile telemedicine platform, and a control group, which received standard outpatient treatment according to the random number method, each consisting of 75 cases. The study assessed changes in constipation symptom scores, the Self-Rating Anxiety Scale (SAS), Short Form 36 Health Survey (SF-36), as well as improvements and patient satisfaction after the intervention.

Results

After the intervention, the intervention group showed significantly lower scores than the control group in areas such as bowel movement frequency, independent defecation, sense of urgency, and defecation time (P<0.05). The intervention group's SAS score was 32.5±9.3, significantly lower than the control group's score of 37.8±10.4 (P<0.05). The scores in physical function, social function, role function, and emotional health were significantly higher in the intervention group than those in the control group (P<0.05). The intervention group experienced shorter time to urge and to complete defecation, and the rate of effective outcomes was significantly higher (P<0.05). Regarding satisfaction, the intervention group rated platform usability, treatment effectiveness, personalized services, and technical support significantly higher than the control group (P<0.05).

Conclusion

Mobile telemedicine interventions demonstrate significant efficacy in remote management for patients with FC. Personalized interventions effectively improve constipation symptoms, mental health, quality of life, and overall patient satisfaction.
