Abstract:
Objective To investigate the changes of the left ventricular systolic and diastolic function after parathyroidectomy in patients of maintenance hemodialysis with secondary hyperparathyroidism.
Methods A total of 33 patients of maintenance hemodialysis with secondary hyperparathyroidism who received parathyroidectomy were selected.Using self control, the patients before surgery were set as the control group, and after surgery as the observation group.Respectively in 1 month before surgery and 6 months after surgery, the intact parathyroid hormone(iPTH), serum calcium, phosphorus, calcium phosphorus product and other biochemical indexes were detected.The interventricular septal thickness and left ventricular posterior wall thickness of left ventricular end diastolic, left ventricular ejection fraction(LVEF), left ventricular end-diastolic volume(LVdV), mitral E/A value, mitral annulus early diastolic peak(Ea)and late diastolic peak(Aa)ratio received by tissue Doppler imaging and left ventricular strain rate(SR)were measured.The matched pairs sample t test was employed.
Results The iPTH, serum calcium, phosphorus, calcium phosphorus product and LVEF, LVdV, mitral E/A valve, SR in the observation group were lower significantly than those in the control group(P=0.000, 0.037, 0.008, 0.000, 0.037, 0.021, 0.026, 0.017, 0.033, all P<0.05). The change of iPTH was most significant in all biochemical indexes, followed by calcium phosphorus product, serum calcium and phosphorus(P<0.05). The changes of mitral E/A valve and Ea/Aa for reflecting left ventricular diastolic function(23.88±10.24, 0.43±13.17)was more obvious than LVEF and SR for reflecting left ventricular systolic function(16.48±3.76, 16.79±4.32)(P<0.05). The difference was not significant between the changes of LVEF and SR(P>0.05). The difference was significant between the changes of E/A and Ea/Aa(P=0.020).
Conclusion The improvement of left ventricular diastolic function in patients of maintenance hemodialysis with secondary hyperparathyroidism after parathyroidectomy is higher than that of systolic function.
