Diagnostic value of double-balloon enteroscopy in patients with negative computed tomography enterography results

Abstract:

Objective

To explore the diagnostic value of double-balloon enteroscopy(DBE)and computed tomography enterography(CTE)in small bowel diseases, and to analyze the predictive factors of DBE positive results in patients with negative CTE results.

Methods

A total of 1013 patients with suspected small bowel disease underwent CTE and DBE examination were enrolled, and relevant data were collected and the difference in detection rate, diagnosis rate and diagnostic value of the two examination methods for small bowel disease was compared.

Results

Taking DBE results as the reference standard, the sensitivity of CTE was 89.6%, the specificity was 49.1%, the positive predictive value was 93.6%, and the negative predictive value was 36.1%.The overall detection rate and total diagnostic rate of DBE were higher than those of CTE.Multivariate analysis showed that age<60 years, anemia, and hypoalbuminemia were predictors of positive DBE results in patients with negative CTE.In patients with negative CTE and positive DBE, microscopic findings were small bowel inflammation, erosion, and ulcerative lesions, followed by small bowel vascular lesions.

Conclusion

Among patients with negative CTE results, patients aged<60 years with anemia and hypoalbuminemia are more likely to be DBE positive.When small bowel mucosal lesions or small bowel bleeding are suspected, if CTE is negative, DBE is still recommended to confirm the diagnosis.
