Abstract:
Objective To investigate the prevalence of hepatitis B virus(HBV)infecting and immunization status of primary school students in Wucun community of Xiamen city, and to provide reference for comprehensive prevention and control strategy of hepatitis B in this region.
Methods A census method was used for a total of 2 722 cases of primary school students in three primary schools in Wucun community of Xiamen city.HBV serum markers were detected by enzyme-linked immunosorbent assay(ELISA), and the immunization status was counted after six months.
Results The positive rate of HBsAg was 2.8%, and the positive rate of HBsAb was 28.4%, both of which showed a decreasing trend with the increase of age, and the age of 6~7 group was significantly higher than the group of age of 10 and above.In the three primary schools, the infection rate of hepatitis B in Wucun Primary School was the highest, and the infection rate of the Second Experimental Primary School was the lowest.A total of 51 cases were found in HBV "Small Positive" , Wucun Primary School with 23 cases, which accounted for almost 45% of the 51 cases.None of cases were found in HBV "Big Positive" . All pupils were vaccinated with hepatitis B vaccine three times in 0, 1 and 6 months after birth, at present, high antibody titer was found in 7 person(0.3%), and low antibody titer was found in 766 person(28.1%). After six months, the actual immunization rates of Wucun Primary School, Second Experimental Primary School and Dongpu Primary School were 28.67%, 62.33% and 10.71% respectively, with a statistically significant difference(χ2=294.9, P<0.01).
Conclusion The three primary school students in Wucun community of Xiamen city have the characteristics of lower HBsAg positive rate, and lower HBsAb positive rate, antibody titer and overall immunization rate, and the serum level of HBsAb has the trend to decrease with the increasing of age.All of the students with HBsAg negative or low level of HBsAb should receive additional immunization, especially those large age group students, which is of great significance to improve the prevention and control of HBV infection among primary school students in Xiamen.
