Diagnostic value of high-frequency ultrasonography and enhanced magnetic resonance for primary hyperdifferentiated hepatocellular carcinoma of different diameters

Abstract:

Objective

To explore the diagnostic value of high-frequency contrast-enhanced ultrasonography(H-CEUS)and contrast enhanced magnetic resonance imaging(CE-MRI)in primary highly differentiated hepatocellular carcinoma of different diameters.

Methods

A total of 128 patients pathologically diagnosed with primary hyperdifferentiated hepatocellular carcinoma in Qingdao Hospital of Rehabilitation University from January 1, 2020 to January 1, 2023 were retrospectively included, including 9 patients who did not meet the inclusion criteria and 5 patients had incomplete data.Finally, 114 patients with primary hyperdifferentiated hepatocellular carcinoma were included as research objects.With pathological results as the gold standard, all patients were examined by H-CEUS and CE-MRI, and the diagnostic efficacy and consistency of the two detection methods were compared, as well as the diagnostic efficacy and consistency of the examination results for lesions of different sizes.

Results

The diagnostic accuracy(98.24%), sensitivity(98.18%)and specificity(100%)of CE-MRI were significantly higher than those of H-CEUS, with statistically significant differences(P<0.05). Compared with H-CEUS, the results of CE-MRI were more consistent with those of pathological gold standard(Kappa=0.804), with statistically significant differences(P<0.05). In primary hyperdifferentiated hepatocellular carcinomas with a diameter≥5 mm, there were no statistically significant differences in the diagnostic accuracy, sensitivity and specificity between H-CEUS and CE-MRI(P>0.05), and there was strong consistency between the two detection results(Kappa=0.743). In the primary hyperdifferentiated hepatocellular carcinomas<5 mm in diameter, the diagnostic accuracy, sensitivity and specificity of CE-MRI were significantly higher than those of H-CEUS(P<0.05), and the results of the two tests were generally consistent(Kappa=0.692).

Conclusion

Both H-CEUS and CE-MRI are valuable in the diagnosis of primary hyperdifferentiated hepatocellular carcinomas of different diameters.However, CE-MRI has better diagnostic efficacy for primary hyperdifferentiated hepatocellular carcinomas with a diameter<5 mm, and is more consistent with the pathological gold standard in diagnosis.
