    Correlation between brain blood supply assessment and clinical treatment of congenital vascular stenosis with magnetic resonance cerebral perfusion imaging

    Objective

    To explore the value of magnetic resonance perfusion-weighted imaging(PWI)in the assessment of blood supply in brain tissue and clinical treatment of congenital vascular stenosis.

    Methods

    Sixty-four patients with congenital vascular stenosis diagnosed with DSA or CTA who have been diagnosed in Hongqi Hospital Affiliated to Mudanjiang Medical College from June 2016 to June 2019 were selected.Regional cerebral blood volume(rCBV), regional cerebral blood volume(rCBF), relative mean transmit time(rMTT)and relative time to peak(rTTP)and other related parameters were obtained from the region of interest after processing with post-processing software.The differences in perfusion between patients with clinical symptoms and those without clinical symptoms were compared, and the perfusion in different areas of patients with clinical symptoms was compared.

    Results

    The differences in perfusion rCBV, rCBF, rMTT and rTTP between patients with clinical symptoms and patients without clinical symptoms were compared.The rCBF value decreased, while the rMTT and rTTP values showed prolongation(P<0.05), and there was no statistical difference in rCBV values between the two groups.The decrease of rCBF and rCBV values and prolongation of rMTT and rTTP values in patients with congenital vascular stenosis were statistically significant(P<0.05).

    Conclusion

    Magnetic resonance perfusion functional imaging technology can effectively evaluate the blood supply of congenital cerebrovascular stenosis in the brain tissue, which is of great significance for the evaluation of clinical blood supply and the choice of treatment options.

    Quantitative evaluation of the efficacy of urate-lowering therapy with spectral CT imaging in early-stage gouty arthritis

    Objective

    To assess the value of material separation technical of spectral CT in evaluating the efficacy of urate-lowering therapy in early-stage gouty arthritis patients.

    Methods

    The clinical and imaging data of 32 early-stage gouty arthritis patients were retrospectively analyzed.Spectral CT scan was conducted on the foot before and after the therapy.Selected uric acid-calcium as basics, two radiologists performed material separation on spectral CT images, and measured the concentration of uric acid around the first metatarsophalangeal joint on uric acid based mapping.The differences of concentration of uric acid and serum uric acid levels before and after treatment were compared by paired t test respectively.Spearman correlation coefficient was used to analyze the association between changes in serum urate levels and spectral CT results.

    Results

    Before the therapy, the mean concentration of uric acid was(1 178.78±11.63)mg/L, mean serum urate level was(421.59±115.27)μmol/L; after the treatment, the mean concentration of uric acid was(1 162.55±9.50)mg/L, mean serum urate level was(309.41±87.59)μmol/L.Compared to the former data, the concentration of uric acid and serum urate level were significantly lower after treatment(P<0.01; P<0.01). The decrease in serum urate levels was positively related to the decrease in the concentration of uric acid on the uric acid based mapping(r=0.53, P<0.01).

    Conclusion

    The spectral CT can evaluate the efficacy of urate-lowering therapy quantitatively through comparing the concentration of uric acid around the first metatarsophalangeal joint in early-stage gouty arthritis, and it can provide more quantitative data for clinical treatment and monitoring.

    At present, the diabetes has become a major threat to public health in our country.It is well known that diabetes and its complications always affect the patient′s quality of life seriously.Patients with early diabetes can control the rise of blood sugar by adjusting their diet, doing more exercise in a moderate amount and using appropriate drugs, but many people go to the hospital when diabetes progresses to a certain extent.Therefore, it is necessary to find an objective method of evaluating the patient′s condition in clinical practice, so as to prophylactic use of therapeutic drugs to avoid complications caused by deterioration of blood sugar.Studies have shown that in the process of type 2 diabetes mellitus(T2DM), the pancreas changes morphologically.With the development of imaging technology, 3D visualization technology has gradually appeared in the medical field.By combining 3D visualization technology with multi-slice CT(MSCT), the differences of pancreas morphological changes in T2DM patients are explored, which is expected to make up the gaps in the diagnosis of T2DM by imaging examination and evaluate the disease process of T2DM patients.

    Feasibility study of evaluation of malignant degree of pancreatic cancer by grey histogram texture analysis based on CT contrast enhancement

    Objective

    To explore the feasibility of grey histogram texture analysis based on CT contrast enhancement in preoperative evaluation of the degree of malignancy of pancreatic cancer, and to evaluate the relationship between grey histogram parameters and pathological grade and differentiated degree of pancreatic cancer.

    Methods

    A total of 49 patients with postoperative pathology proven pancreatic cancer including high differentiation(27 cases)and low-moderate differentiation(22 cases)were analyzed retrospectively.Region of interest(ROI)was chosen at axial CT images with maximum enhancement of lesion and grey histogram analysis was performed using Mazda software.The histogram parameters were analyzed statistically in these two groups.Receiver operating characteristic(ROC)curve was established for parameters to compare diagnostic performance.In addition, the correlation was tested between parameters and groups.

    Results

    In the 9 parameters extracted from grey histogram analysis among the two groups, the mean value, Perc.01%, Perc.10%, Perc.50% had statistical differences(P<0.05), but variance, skewness, kurtosis, Perc.90%, Perc.99% had not statistical differences(P>0.05). Spearman correlation analysis showed that the mean value, Perc.01%, Perc.10%, Perc.50% were positively correlated with the differentiated degree(r values were 0.04, 0.29, 0.32 and 0.33 respectively, P<0.05). When evaluating malignant grade of pancreatic cancer, the diagnostic efficiency of the mean value was the highest(AUC=0.695). When the cutoff value was 147.27 for mean value, the sensitivity and specificity were 86.4% and 44.4%.When the cutoff value was 117 for Perc.01%, the sensitivity and specificity were 95.5% and 40.7%.When the cutoff value was 120 for Perc.10%, the sensitivity and specificity were 72.7% and 63%.When the cutoff value was 146 for Perc.50%, the sensitivity and specificity were 86.4% and 44.4%.

    Conclusion

    The characteristic parameters of gray histogram texture analysis based on CT enhanced images are different between high and low-moderate grade pancreatic cancer, providing a new method for preoperative evaluation of the degree of malignancy of pancreatic cancer.

    Effect of preoperative MRI on surgery program of patients with breast invasive lobular carcinoma by propensity score matching

    Objective

    To evaluate the effect of magnetic resonance imaging(MRI)examination on the treatment of patients with breast invasive lobular carcinoma(ILC)by using propensity score matching, and to study the relationship between preoperative breast MRI and the surgical outcomes of ILC patients.

    Methods

    The clinical and imaging data of 210 patients with ILC who underwent surgery from January 2006 to December 2016 were retrospectively analyzed who had been admitted to Beijing Hospital of Integrated Traditional Chinese and Western Medicine and Beijing Shangdi Hospital.61.9% of patients(130/210)underwent MRI examination.The MR detection rate of undetected lesions in mammography and ultrasonography was calculated and the patients′ treatment processes were analyzed.After the propensity scores matching, 34 patients with 1∶1 matched ratio, the differences among 14 basic characteristics and tumor characteristics in patients undergoing preoperative MRI(MRI group)and patients without preoperative MRI(control group)became consistent.Then the surgical results were compared.

    Results

    Of the 130 patients in the MRI group, 34.6%(45/130)had occult lesions; 62.2%(28/45)had malignant lesions.In 24.6% of patients(32/130), the surgical plan changed due to preoperative MRI examination, and 91.1%(41/45)of the results were consistent with pathology.In the propensity score matching analysis, breast MRI was associated with a lower incidence of re-operation, with an odds ratio(OR)of 0.135, P<0.05.The first mastectomy(OR=0.736, P=0.663)was similar to the final mastectomy(OR=0.691, P=0.141)and there was no statistical correlation with preoperative breast MRI.

    Conclusion

    Preoperative MRI can be used to detect occult malignancies and significantly reduce the incidence of recurrent surgery without increasing the rate of mastectomy in patients with ILC.

    Value of perfusion parameters of 320-detector-row dynamic volume CT in the evaluation of differentiation of advanced gastric cancer

    Objective

    To evaluate the features and differences of various CT perfusion parameters on pathologically proven gastric cancer with labeled differentiations.

    Methods

    A retrospective analysis of the CT perfusion parameters was performed on 80 patients with gastric cancer who were treated by surgery and confirmed by pathology from September 2014 to February 2016 in First Affiliated Hospital of Zhengzhou University.All cases were underwent the perfusion study on the Toshiba 320-detector-row dynamic volume CT.The 80 lesions were layered according to the enhancement degree.The lesion with hypo enhancement was defined as the first layer, and the second layer was right below to it.The perfusion parameters of arterial flow(AF), blood flow(BF), blood volume(BV)and clearance(CL)of the two layers and the whole layers were measured.The inter-group difference of the perfusion parameters was analyzed on the first two layers and whole layers in all confirmed cancer cases with different differentiation adenocarcinoma, while the intra-group difference was analyzed using statistical method.

    Results

    For the 46 cases with poorly differentiated adenocarcinoma, the parameters of AF, BF, CL in the first layer were significantly higher than those in the second layer(P=0.000, P=0.024 and P=0.010), but there was no difference of the BV(P=0.204). For the 34 cases with well-differentiated adenocarcinoma, the parameters of AF, BV, BF in the first layer were significantly higher than those in the second layer(P=0.001, P=0.001 and P=0.000), and the CL had no difference(P=0.145). There was no difference of perfusion parameters of the first two layers and whole lesion layers in between the poorly differentiated and well-differentiated groups(all P>0.05).

    Conclusion

    The distribution of blood vessels in gastric cancer tissues with different degrees of differentiation is different, which means they have inconsistent perfusion parameters.

    Relation between carotid artery elasticity and left ventricle diastolic function about cigarette smoking evaluated by echo tracking

    Objective

    To evaluate the application value of echo tracking(ET)on carotid artery elasticity of smoking people, and combined with heart diastolic function to investigate the relation of carotid artery elasticity and left ventricle diastolic function.

    Methods

    The two common carotid arteries of 31 cases active smoking people and 30 cases of non smokers were detected by ET, and the maximum systolic blood vessel diameter(Ds), diastolic minimum diameter(Dd), elastic coefficient(Eρ), stifness paramet(β), swelling index(AI), compliance(AC)and pulse wave velocity(PWVβ)were measured.In addition, the cardiac diastolic function indexes such as mitral valve pulse spectrum E/A, tissue doppler E′/A′, pulmonary vein S peak/D peak(Vs/Vd), AR wave duration time(ARD), left ventricular end diastole diameter(LVDd), left ventricular mass index(LVMI).

    Results

    ①The differences of Dd, Ds, β, Eρ, AI, PWVβbetween active smoking group and the normal group were not statistically significant(P>0.05). ②The LVDd, LVMI and ARD of active smoking group were higher than those of the normal group(P<0.05). ③The differences of E/A, E′/A′, E/E′and Vs/Vd between the two groups were not statistically significant(P>0.05). ④Correlation analysis showed that ARD had positive relation to Eρ(P<0.05), and E/E′had negative relation to AC and positive relation toβ, Eρ, PWVβ(P<0.05)in active smoking group.

    Conclusion

    Heart diastolic function can reflect cigarette smoking people The blood vessels elastic function of smoking people is positively correlated with heart diastolic function, and heart diastolic function decreases with the decrease of vascular elasticity.

    Oral metformin and the risk of colorectal adenoma: a systematic review and meta-analysis

    Objective

    Some studies indicate that oral metformin can significantly decrease the risk of colorectal cancer, but it is unclear whether it protects against colorectal adenoma.So we further clarify the correlation between the two factors by means of Meta analysis.

    Methods

    We searched the control trials meeting the inclusion criteria through the PubMed, EMBase, the Cochrane Library, SinoMed published before June 2017.Two authors independently retrieved the databases and extracted the data.Stata 12.0 software was used to analyze the data.

    Results

    Ten studies were included in this Meta-analysis.The results indicated that oral metformin could decrease the risk of colorectal adenoma(OR=0.74, 95% CI(0.58-0.90), I2=61.9%, P=0.05). We divided the papers into two subgroups: case-control study and retrospective cohort study, and the results were as follows: OR=1.04, 95% CI(0.77-1.31), I2=0, P=0.372; OR=0.66, 95% CI(0.49-0.84), I2=65%, P=0.009.

    Conclusion

    Oral metformin can decrease the risk of colorectal adenoma.
